Mid-back pain is often overlooked, yet it can significantly affect posture, breathing, sleep and upper limb movement. It may present as stiffness between the shoulder blades, rib discomfort, aching across the back or sharp pain with twisting or deep breathing.

Because the thoracic spine links the neck, shoulders and lower back, dysfunction here can influence multiple regions. When symptoms persist, it is rarely because the area is “fragile.” More often, the mechanical or movement-related driver has not been precisely identified.

At Solent Specialist Physiotherapy, thoracic spine assessment and rehabilitation form part of our advanced spinal service within our Fareham clinic.

Why mid-back problems need specialist care

The thoracic spine is designed to provide stability while allowing controlled rotation and expansion of the rib cage for breathing. Pain in this region may arise from joint irritation, rib joint restriction, muscular overload, breathing pattern changes or altered movement strategies.

Symptoms are frequently attributed solely to “poor posture,” yet posture alone rarely explains persistent discomfort. Similarly, imaging findings such as mild degenerative change do not always correlate with symptom behaviour.

Thoracic pain may also be influenced by cervical or lumbar mechanics, shoulder loading patterns or protective guarding following previous injury.

Specialist assessment evaluates joint mobility, rib movement, breathing mechanics, muscular coordination and load tolerance. This allows treatment to target the true mechanical contributor rather than applying generalised stretching or strengthening.

Accurate differentiation reduces recurrence and restores confidence in movement.

Common thoracic (mid-back) conditions we manage:

Persistent or recurrent mid-back pain

Rib-related pain and joint restriction

Thoracic stiffness between the shoulder blades

Breathing-related thoracic discomfort

Postural and desk-related thoracic pain

Thoracic outlet–related symptoms

Referred pain from cervical or lumbar regions

Facet Joint Osteoarthritis→

Post-injury or post-surgical thoracic rehabilitation

If you’re unsure what’s driving your mid-back symptoms, a specialist assessment can provide clarity and reassurance.

What makes our thoracic spine service different

Thoracic spine rehabilitation within our Fareham clinic is delivered by Band 8–level specialist physiotherapists working at advanced clinical level within the NHS and private practice.

Band 8 physiotherapists represent the highest level of advanced clinical practice within the NHS, involving complex diagnostic decision-making, management of persistent or post-operative conditions, and close collaboration with medical and surgical teams.

Many individuals attending with mid-back pain have previously received general advice focused on posture correction alone. While posture may contribute, persistent symptoms often involve more complex interactions between joint mobility, breathing mechanics and muscular coordination.

Specialist evaluation examines how your thoracic spine behaves during rotation, lifting, sitting, overhead activity and breathing. It considers irritability profile, rib motion, movement control and load distribution across the upper trunk.

Rehabilitation is structured around restoring mobility where needed, improving rib and breathing mechanics, refining movement control and gradually increasing tolerance to functional load.

For joint-driven pain, management focuses on restoring segmental mobility and balanced activation. In rib-related discomfort, treatment addresses both joint mechanics and breathing pattern integration. For persistent mechanical pain, progression builds resilience through graded load exposure rather than avoidance.

Programmes are individualised and progression is guided by symptom behaviour and functional tolerance rather than fixed timelines.

Care is guided by advanced clinical reasoning and specialist experience rather than protocol-driven or generic rehabilitation approaches.

Where appropriate, we can:

• Request and interpret imaging when clinically indicated
• Correlate scan findings with mechanical behaviour rather than report wording alone
• Liaise with local services regarding interventional options such as facet injections if appropriate

This integrated approach supports durable improvement and reduces recurrent flare cycles.
